Community HealthChoices (CHC) Waiver:

  • Available for adults the age 21 or older with a physical disability. The individual must meet the nursing facility’s level of care criteria to receive long-term services and supports in own home or another community setting.

    OBRA Waiver:

  • Available to individuals with developmental physical disabilities, the disability results in at least 3 substantial functional limitations, the disability manifested prior to age 22, person is age 18 or older, the disability is expected to continue indefinitely, and person requires ICF/ORC level of care.

    ACT 150:

  • Pennsylvania's Act 150 Program enables adults 18 through 59 years of age who are mentally alert and have physical disabilities to perform activities of daily living (such as eating, personal hygiene, and transporting themselves).

    Adult Autism Waiver:

  • The Adult Autism Waiver (AAW) is a 1915(c) Home and Community-Based Services (HCBS) Medicaid waiver designed to provide long-term services and supports for community living, tailored to the specific needs of adults age 21 or older with ASD.

  • We are an active provider under the Pennsylvania Options Program

  • Non-medical in-home care services focuses on helping seniors and people with disabilities with what are known as “activities of daily living” so that they can remain living safely at home in the community. Types of non-medical home care services include: bathing, grooming, transferring, toileting, meal preparation, light housekeeping, medication reminders and escorting to doctors’ appointments and errands.

  • There are a variety of ways to pay for non-medical home care, including through a state’s Medicaid Waiver Program, County OPTIONS Programs, long-term care insurance and private pay.

  • These are state programs designed to provide services to people who would otherwise be required to enter an institution, nursing home or hospital to receive long-term care services. Medicaid Waivers allow these individuals to receive needed non-medical home care services at home without having to enter an institution. These services are provided at NO cost to qualified recipients.

  • It can take up to 8 months.
